How to bid a medical office cleaning job

Medical offices pay more than plain offices for a reason: exam rooms, restrooms and waiting areas take longer, need hospital-grade disinfectant, and the client cares about consistency more than price. Here is how to cost one, with a worked example.

What changes versus a regular office

Your production rate drops. A plain carpeted office might clean at 3,000-4,000 sq ft an hour; a clinic with exam rooms, sinks and high-touch surfaces is often closer to 2,000-2,500. Supplies cost more (EPA-registered disinfectants, more liners, more paper). Walk every room on the site visit and ask about sharps, biohazard and lab areas: those are usually handled by a licensed medical waste service, not your crew, so write that exclusion into the proposal.

The worked example

A 4,000 sq ft medical office cleaned 5 nights a week. Assumptions (yours will differ): production rate 2,200 sq ft/hr, loaded wage $20.40/hr ($17 base plus 20% burden), supplies $65/month, overhead 10% of labour, 25% margin.

Hours per visit: 4,000 / 2,200 = 1.82. Visits per month: 5 x 52 / 12 = 21.67. Labour hours per month: 1.82 x 21.67 = 39.4.

Labour: 39.4 x $20.40 = $804. Supplies: $65. Overhead: $80. Total cost: $949 a month.

Price on a 25% margin: $949 / 0.75 = $1,265 a month, about $0.32 per sq ft per month, or $32 per labour hour billed.

What goes in the proposal

List the scope room by room (exam rooms, waiting area, restrooms, staff areas), the disinfection standard you follow, what you exclude (medical waste, sharps), the visit schedule, and the monthly price. Clinics compare proposals on reliability, so name who supervises and how missed items get fixed.
